NDIS Line Item: 15_621_0128_1_3 – Assessment, Recommendation, Therapy or Training – Social Worker
Rate: $193.99 per hour
Accessing support through your NDIS plan:
Ensure your NDIS plan includes Capacity Building – Improved Daily Living funding
Choose us as your provider (plan-managed or self-managed)
Complete a service agreement
Invoices are sent directly to your plan manager, or to you if self-managed
